The way UHNWs think about healthcare is changing. Increasingly, it is not simply something to turn to when illness strikes, but something to be actively managed – and, for some, invested in.
The growing interest in longevity has helped accelerate this shift. High-profile biohackers such as Brian Johnson, who has invested millions in attempts to reverse his biological age, have brought the pursuit of longer, healthier lives into the mainstream. Private doctors and GPs are responding, expanding their practices beyond traditional primary care to encompass preventative medicine, health screening, wellness and healthy ageing.
‘I am passionate about helping clients understand their health at a deeper level, empowering them with the knowledge and confidence to make informed decisions that support better health for years to come,’ says Dr Asha Katwa of Preventicum, a health-screening clinic offering a range of scientifically tuned assessments and tests.

Yet for all the advances in preventative medicine, the role of the private GP remains rooted in something more fundamental: trust. From routine check-ups to serious health scares, private doctors & GPs can be a constant presence through some of their patients’ most challenging and sensitive moments.
The GPs featured in the 2026 edition of the Spear’s Index provide everything from consultations, vaccinations and prescriptions to minor illness management, while also offering access to a wider network of leading consultants and private hospitals when specialist care is required. For many patients, it is also the continuity of that relationship that sets private general practice apart. ‘The reason I went into general practice is the continuity of care,’ Dr Tim Lebens of The Wellington Hospital tells Spear’s. ‘If you feel you can trust your doctor, then you’re halfway there to getting better.’
‘We are family-friendly; we give patients a lot of time. People have my mobile; they can get one of us 24/7,’ Dr Tim Ladbrooke of Courtfield Private Practice said. That emphasis on personalised care extends across the wider spectrum of private medicine. The specialists featured this year span disciplines from orthopaedics and sports medicine to cancer treatment.
Dr Sam Botchey, a new addition to this year’s index, specialises in musculoskeletal wellness and longevity. His practice, which he describes as being at ‘the sharp end of medicine’, brings together treatments including stem-cell injections, MRI and whole-body scans for patients experiencing chronic pain or injury. Meanwhile, Dr Andrew Goldberg carries out around 100 ankle replacements each year with the use of his pioneering patient-specific 3D-printing technology. Rather than reshaping a patient’s bones to fit a standard implant, the replacement is designed around the patient’s own anatomy.
Trends in 2026
The rise of GLP-1’s
Weight management has become an increasingly popular area of medicine for the practitioners represented in our list. In 2026, a PwC report found that 9 per cent of UK adults, equivalent to around five million people, had used a GLP-1 medication, with a further 18 per cent saying they would consider using one in the future.
Among private practitioners, these medications are increasingly being incorporated into structured weight-management programmes. Dr Catrin Bevan of The London General Practice runs weight-loss programmes for menopausal women, including prescribing medications such as Ozempic and Mounjaro where clinically appropriate.
‘People are seeing success and losing weight very well, particularly when coached through how to diet and exercise by us. We’ve got quite a robust programme,’ she tells Spear’s.

International care
For international patients, the ability to access trusted medical care across borders is another attraction. ‘I have a large proportion of international patients,’ says Lebens. ‘I do virtual consultations in the afternoon, which tends to be best if [the patients] are in the States, India, Asia, Nigeria and the Middle East.’

‘We have clients from all over the world,’ Dr Willoughby Morgan of King-Lewis Family Practice explains. ‘We can arrange consultations as needed wherever they are, from Bermuda to the Middle East to Europe.’ The result is a model of private medicine that increasingly combines the traditional virtues of the family doctor: trust, continuity and discretion with the modern technology of virtual appointments.

Methodology
Each year, the Spear’s Research Unit reassesses and refreshes its rankings of the leading providers in each sector by gathering data from and about the advisers and firms themselves, assessing submission forms, collating nominations, carrying out peer reviews, reviewing data from third-party sources, gathering references and recommendations, canvassing experts and conducting hundreds of interviews.
Advisers are evaluated using a proprietary scoring system that assigns different weightings to certain attributes. These scores feed directly into each new set of rankings in the Spear’s Indices. Each of these indices are published first online (according to the research calendar) and then in print. Print publication takes the form of the annual Spear’s 500 directory, which includes the top advisers in every index.

Each featured adviser is profiled on spears500.com. The site allows users to search the Spear’s database of more than 4,000 entities to find one (or more) to meet their specific requirements by filtering for specific attributes such as an adviser’s location, their specialist expertise and information about their client base.
The best private doctors & GPs: some names to know
Andrew Goldberg
- Focus: Foot and ankle surgery
- Position: Consultant orthopaedic surgery
- Practice: HCA Healthcare
Andrew Goldberg OBE is a highly regarded consultant orthopaedic surgeon and one of the top specialists in the UK for foot and ankle conditions.
‘I do clinics every day of the week, so basically people can see me and get seen for follow-up either the same day or the next day,’ he tells Spear’s. ‘We’re really a one-stop shop.’
His clients include elite athletes, high-net-worth individuals, top executives, business owners and celebrities; he is also one of a few orthopaedic surgeons who takes on both children and adults as part of his practice. His clinics are based at The Wellington Hospital in St John’s Wood; in Canary Wharf; and in Elstree, Hertfordshire, as part of HCA Healthcare.
In addition to his private practice, Goldberg, who was awarded an OBE in 2011 for services to medicine, is an active researcher, at the forefront of developments such as 3D printing for use in ankle replacements and regenerative treatments. Considering the whole patient rather than rushing to follow a surgical path, Goldberg estimates that he avoids surgery on more than 85 per cent of the patients he sees.

Garry Savin
- Focus: Health assessments
- Position: Founder and GP
- Practice: Lumen
‘We regard ourselves as the Rolls-Royce of health assessments in Europe,’ says Dr Garry Savin, who leads a team of dedicated medical professionals at the Lumen clinic in Harley Street. His primary aim is the prevention and early detection of disease.
His practice acts as a ‘one-stop shop’ for clients who are seeking to detect problems that might indicate future risks, such as heart disease or the very early stages of conditions such as common cancers. Clinicians perform a wide range of health screening tests, and patients can expect to receive tests and results on the same day. Should a problem or condition be detected, Savin can make suggestions for lifestyle changes, or refer patients directly to a specialist where they are then treated as non-invasively as possible.
In 2025, Savin launched BrainKey, an innovative type of cognitive assessment. ‘We can measure each individual part of the brain in 3D,’ he tells Spear’s. ‘It’s a very precise way of quantifying the brain over time to see if it’s behaving oddly.’

Paul Ettlinger
- Focus: Preventative medicine
- Position: Founder and GP
- Practice: The London General Practice
Dr Paul Ettlinger is founder and medical director of The London General Practice on Harley Street. The doctors here offer a comprehensive service, from initial GP appointments to referrals to an international network of specialists for onward care.
Ettlinger has particular interests in men’s health and preventative medicine, providing assessments for individuals and corporate clients. His paper on the importance of personal continuity in general practice was published in the British Medical Journal.
The practice aims to provide same-day appointments, for a minimum of 30 minutes, to allow its consultants to conduct a comprehensive evaluation of a patient’s health.
Prior to his role at The London General Practice, Ettlinger was a principal in a Kensington, Chelsea and Westminster NHS practice and lectured at St Mary’s Hospital Medical School at the University of London.

Antionette Sarasin Gianduzzo

‘Since I was very young, I have always been interested in health and how it can influence the way you think, perform in school or sports and even how ambitious you are,’ says Dr Antoinette Sarasin Gianduzzo, who founded and continues to be the director of the Biomolecular Restoration and Rejuvenation (BIO-R), nutrition and lifestyle team at Kusnacht Practice. Her primary aim is ensuring ‘vitality in the body and in the brain’ of each patient who passes through the clinic.
An innovative approach to medicine, BIO-R is ‘an in-depth, innovative procedure that corrects the imbalances and stress factors in complex biochemistry to strengthen physical and emotional wellbeing, successful weight management, rejuvenation and anti-ageing’.
Her patients are primarily dealing with addiction issues, burnout or serious mental health problems and require bespoke treatment plans.
‘We tailor each programme for the patient,’ Sarasin Gianduzzo explains. ‘We don’t just check them once; they receive continuous monitoring twice or three times a year going forward.’
